Privacy Policy
Longevity Perform ("we," "us," or "our") respects your privacy. This Privacy Policy explains what information we collect through this website and our booking tools, how we use it, and the choices you have. It applies to longevityperform.com and to information you share with us by phone, email or text in connection with our services.
Who we are
Longevity Perform is a chiropractic and performance care practice operated by Longevity Perform LLC, a Florida business located at 7500 NW 69th Ave, Unit R7, Medley, FL 33166. Care is provided by Dr. Jose Sierra Prado, D.C., a chiropractic physician licensed in Florida. You can reach us at (939) 308-1870 or drsierra@longevityperform.com.
Information we collect
- Concierge call requests. When you request a concierge call on this website, we collect your first and last name, phone number, email address, a short description of what you need, and your preferred call date and time.
- Online booking. Studio appointments are booked through Jane App, our practice management and scheduling platform. Jane collects the information needed to book your visit, such as your name, contact details, appointment details and a payment card to hold your appointment. That information is handled by Jane on our behalf and is also subject to Jane's privacy policy.
- Payments. Card payments are processed by our payment processors (such as Stripe, through Jane or directly). We do not store full card numbers on our website. Card data is handled by processors that follow the Payment Card Industry Data Security Standard (PCI DSS).
- Communications. If you call, text or email us, we keep a record of that communication.
- Website usage data. Like most websites, our site and hosting provider may automatically collect technical information such as your IP address, browser type, device, pages visited and referring site. We may use cookies and similar tools for basic site functions (for example, remembering your language choice) and, if enabled, for analytics.
Health information
Information about your health, symptoms and treatment is confidential. Patient records are kept confidential as required by Florida law, including Section 456.057, Florida Statutes, and are not disclosed without your written authorization except as permitted or required by law. Where the federal HIPAA Privacy Rule applies to the services you receive, our Notice of Privacy Practices also applies; you may request a copy at any time.
Please do not send detailed medical history through the website form or regular email. Share only what we need to understand your request, and we will discuss the details with you directly.
How we use information
- To respond to your requests, schedule and confirm appointments, and call you at the time you selected.
- To provide, document and bill for care.
- To send appointment reminders and service-related messages.
- To operate, secure and improve our website.
- To comply with legal, professional and regulatory obligations.
Calls, text messages and email
By submitting your phone number and email, you agree that we may contact you by phone call, text message or email about your request, your appointments and your care. These messages are not marketing. We will only send marketing messages if you separately agree to receive them. Message and data rates may apply. You can reply STOP to any text message to opt out of texts, and we will honor your request promptly. Consent to receive messages is not a condition of receiving care.
How we share information
We do not sell your personal information, and we do not share it for third-party advertising. We share information only:
- With service providers that help us run the practice, such as Jane App (scheduling and records), payment processors, our website host, email and calendar providers, under obligations to protect it;
- When you authorize us to, for example with a coach, trainer or other provider you name;
- When required or permitted by law, such as to respond to a court order or to protect health and safety.
Cookies and analytics
You can block or delete cookies in your browser settings; the site will still work. If we use analytics or advertising tools, we configure them so that they are not placed on our booking and concierge request pages and do not receive health information.
How we protect information
We use reasonable administrative, technical and physical safeguards, including encrypted (HTTPS) connections. No method of transmission or storage is completely secure. If a security breach affects your personal information, we will notify you as required by the Florida Information Protection Act (Section 501.171, Florida Statutes) and other applicable law.
How long we keep information
We keep patient records for at least as long as Florida law requires for chiropractic records. Concierge call requests that do not become patient records are kept only as long as needed to respond and follow up, then deleted.
Minors
This website is not directed to children under 13, and we do not knowingly collect information from them online. Appointments for patients under 18, including youth athletes, must be requested and booked by a parent or legal guardian.
Your choices and rights
You may ask to see or get a copy of your records, correct information, or stop texts and emails at any time. Contact us at drsierra@longevityperform.com or (939) 308-1870. We will respond within the time required by law.
Changes to this policy
We may update this policy from time to time. The "Last updated" date at the top shows when it last changed.
