These Terms of Use govern your use of longevityperform.com (the "Site") operated by Longevity Perform LLC doing business as Longevity Perform. By using the Site you agree to these terms. If you do not agree, please do not use the Site.
Not medical advice
The content on this Site is for general information and education only. It is not medical advice, diagnosis or treatment, and it does not create a doctor–patient relationship. A doctor–patient relationship begins only after you are evaluated in person by Dr. Jose Sierra Prado, D.C.. Always seek the advice of a qualified health care provider about a medical condition.
If you have a medical emergency, call 911 or go to the nearest emergency room. Do not use the Site, email or text messages for urgent or emergency needs.

Every person responds differently to care. Testimonials, reviews and athlete features on this Site describe individual experiences and are not a promise or guarantee of any outcome. No specific result is guaranteed.
Appointments, fees and payment
- Prices shown on the Site are in U.S. dollars and may change. The price that applies is the one confirmed when you book.
- Longevity Perform is an out-of-network, self-pay practice. Payment is due at the time of service unless otherwise agreed.
- A valid payment card is required to hold studio appointments booked online. Your card is not charged at booking.
- Concierge visits are quoted individually after a phone consultation, based on location, travel and treatment. The quote is confirmed with you before the visit is scheduled.
- Cancellations, no-shows and refunds are governed by our Cancellation & Refund Policy.
- If you are uninsured or not using insurance, you have the right to a Good Faith Estimate of expected charges.
Third-party services
Online booking is provided through Jane App, and payments may be processed by Stripe or other processors. Links to third-party sites, such as Google, Instagram, Facebook and TikTok, are provided for convenience. We are not responsible for third-party sites or services, which have their own terms and privacy policies.
Acceptable use
You agree not to misuse the Site, including submitting false information, attempting to access systems without permission, or interfering with the Site's operation.
Intellectual property
The Site's content, including text, logos, photos and videos, belongs to Longevity Perform or is used with permission, and may not be copied or reused without our written consent.
Disclaimer and limitation of liability
The Site is provided "as is" without warranties of any kind. To the fullest extent permitted by law, Longevity Perform is not liable for indirect, incidental or consequential damages arising from your use of the Site. Nothing in these terms limits any liability that cannot be limited under Florida law.
Governing law
These terms are governed by the laws of the State of Florida. Any dispute relating to the Site will be brought in the state or federal courts located in Miami-Dade County, Florida.
